The fill-in forms using Microsoft Word or Adobe PDF forms make it easy to complete your application or report on your computer. Download or open the fill-in form document and fill in the form using Microsoft Word. Enter information in the shaded “fields” of the form. To move from field to field, use your mouse, the arrow keys or press Tab. Use your mouse or the spacebar to mark and unmark a checkbox. You may email the completed word form as an attachment.
To e-mail a word form: left mouse-click: file > send to > mail recipient as an attachment.
You may save a copy to your computer as well.
To email a PDF form, simply select the submit button and the file will automatically be attached to an outgoing email to the designated recipient.
